The following Example has been taken out of IMT viewer so the formatting might be off.  I did not changed mission information, only names to protect myself and the people I have received the information from.

I will attempt to give a little insight to the entire process if I know it.

MAJOR ACHIEVEMENTS:
- Leads by example; diligent professionalism with an extreme work ethic who proved invaluable to the mission 
- Always ready; mobilized for Operation Iraqi Freedom (OIF) from June 03 to January 04; first to volunteer
- Deployed on short notice for 190 days to USCENTCOM supporting # Air Expeditionary Mobility Squadron 
- Key player on the Network Control Center (NCC); SrA <LNAME> performed system hardware and software 
  operations; administration and management that supported more than 1500 people in 6 months
- Commitment excellence to mission was instrumental in guaranteeing 24/7 availability to the war fighter
- Service with a smile; expertly handled over 600 help desk assignments, loading programs, files, and databases
- Configured and engineered a network firewall during change-over of personnel--securing the network
- Responded to trouble calls and console messages and signals isolating faults, determining causes and
  recovering computers from malfunctions due to hardware and software and communications failures
  -- Ensured 47th Combat Support Hospital received secured classified network service within an hour after
     cable was destroyed by heavy equipment--his efforts were key to restoring service; major accomplishment
     --- Vital for incoming army personnel casualties being process for evacuations for additional treatment

- Worked diligently to contain a virus outbreak on the network to ensure minimal downtime -- four different 
  virii struck within a two week period -- all considered, able to keep 98% uptime during the attack

LEADERSHIP ABILITIES AND MANAGEMENT EXPERIENCES:
- Primarily responsible for nearly $2 million worth of deployed communications equipment and resources
- Produced detailed maps of Camp Swampy, Camp Swampier and Air Traffic Ops Center network infrastructure
  -- Meticulously traced, identified, labeled and tested all Network Control Center (NCC) communications 
      equipment and documented for latest anti-virus update; ensured all patches were installed and working
- Led deployed team as computer and printer maintenance primary technician during period when the site was 
  short of working equipment and with a lack of ample parts and supplies--resourceful in acquiring needed parts
- Provided training to army personnel protecting the network and proper procedures for reporting violations
- Outstanding communications operator whose skills and management experience well exceeds those of his peers

OTHER ACCOMPLISHMENTS:
- While deployed, volunteered to DJ the music for the Christmas and New Years party; received high accolades
- Duly recognized as the Outstanding Performer for the Month of Sept 2003 for his contributions to the mission 
-- Received a coin from Brig General Halftrack, who was visiting the area of responsibility during this time
-- Furthermore, Maj General Brass was also present and stated, "...if Amn <Lname> would pursue becoming a pilot 
    I would write a personal recommendation for him"; an incredible compliment and honor 
- Presented a coin from the commander of the Army Field Hospital for providing outstanding support in fixing 

NOTE:

  VERY Good package!  Writer used persons name, that is something normally frowned on.  Besides that, this is a very solid package, and the board probably never noticed it.
