The following Example has been taken out of IMT viewer so the formatting might be off.  I did not changed mission information, only names to protect myself and the people I have received the information from.  If you have anything you would like to submit, please do so.  If you have found this information useful, please let me know. SUSTAINED JOB PERFORMANCE

- Top-notch maintainer; exhibited unsurpassed initiative and technical skills--#1 of 59 airmen in the Flight

- Performed flawlessly as member of 4 man launch crew during Operational Readiness Inspection Jul 04;
  expertly performed 5 fuel leak evaluations and repairs--enabled "OUTSTANDING" generation of 24 aircraft

- Dispatched to an F-15 aircraft with a leaking engine to airframe manifold (ETAM); keen observation led to
  discovery/replacement of leaking manifold up-line of ETAM; saved $5K in potential replacement cost

- Dedicated--completed 40 of 234 external fuel tank lug replacements; ensured on-time completion of TCTO 508

- Competent technician; attained a superb 100% pass rating on 8 Quality Assurance evaluations and inspections

- Troubleshot an F-15 experiencing an external tank transfer imbalance; quickly identified/replaced a cracked
  air standpipe in minimal time; allowed sortie generation in support of Operation NORTHERN GUARDIAN

JOB KNOWLEDGE

- Raised the bar! Knocked out all three volumes of CDC early; scored an impressive 95% on end of course test

NOTE:

  Great package!   Excellent flow, a good example for others to use.  On a side note, you can tell how packages change every year.  If you look at the 2003 and 2005 examples you will get the idea.
