The following Lance P. Sijan USAF Leadership Example has been taken out of a FORMFLOW document.  I have changed mission information to protect myself and the people I have received the information from.  I will attempt to give a little insight to the entire process if I know it.

In preparing this award for the web I found a couple of typos.  Nothing kills points faster then misspellings in an award package.  Use the spell checker, it might make all of the difference.  I liked this award, there is lots of information.  I personally found COMMUNITY INVOLVEMENT to be the strongest section.


SCOPE OF RESPONSIBILITY

COMMUNITY INVOLVEMENT
- Regular blood donor for United Blood Services; gave blood at five blood drives this period--he also is
  registered with the DoD marrow donor program; wears his heart on his sleeve--willing to help anyone
- Participated in a 24-hour walk-a-thon during the American Heart Foundation's Walk for Life event
- Unique member of the local school board's Community Advisory Council for special education; a 3
  year member...worked with teachers, parents and counsel members to improve local public education
  -- Addressed the educational requirements and social development of local area special needs children
  -- Spokesman for Beale's families with school aged children at the California Regional Military Child
     Conference; a high honor...1 of 4 airmen from base selected; represented views of the base families
- DGS-? Top-4 Enlisted Club President; set the direction of activities designed to improve unit morale
  -- Menu coordinator for the DGS-?'s first Dining Out...a demanding chore requiring expert planning
- Grill Master; averages one cook out a week for unit functions...retirements, fund raisers, VIPs, etc.
  -- Earned the DGS-? Booster Club and Top-4 organization more than $1K during this period of time
- Led the unit in Christmas cheer...set up and ran the children's Christmas party and cooked Christmas
  dinner for the mission shift personnel; the care for his fellow airmen is self-evident...sets the example
- Set up the DGS-?'s children's Easter egg hunt; 80+ children and adults enjoyed searching for candy
- Helped cook Thanksgiving dinner for the whole unit--a lot of hard work and an outstanding success
- Trekked over 100 miles on a sponsored Bike-Around-the-Buttes Diabetes fund-raiser; raised $1,105
  by getting sponsors for his 1-day trip around a small mountain range; visibility attracted other riders
- Assisted the Sutter-Yuba counties gleaners food bank...sorted out seasonal produce items rejected by 
  packers due to appearance or color but perfectly all right to eat...food given to local needy families
- Manages community's monthly United States Department of Agriculture's food give-away program 
  -- Distributed free food items to young airmen with families...gave them extra help to make ends meet
- Worked the weekly food give away program at the base chapel; a squadron sponsored program that
  collects perishable goods from the base commissary which can not be sold but are suitable for eating
- Promotes fitness to Nth degree...bikes to work and leads the commander's Physical Fitness program


Final Note:

  This is a long package.  As are most two page 1206s.  What I found interesting was the persons name was used in the package...
